About the Role
Our mission revolves around understanding health trends, identifying risk factors, and addressing barriers to wellness—all while working closely with patients remotely. In this role, you'll serve as the critical bridge between data-driven insights and personalized patient care, leveraging your clinical skills to promote preventative health, manage chronic conditions, and empower individuals to take charge of their wellness.
Key Responsibilities
Preventive and Proactive Care
- Conduct telephonic or virtual assessments to identify high-risk individuals.
- Educate patients about preventive measures, screenings, and the management of chronic conditions.
- Guide members through lifestyle changes that promote long-term health.
Individualized Health Support
- Create and adjust care plans in response to evolving patient needs.
- Foster understanding by using patient-friendly language and visual aids when appropriate.
- Provide motivational interviewing and coaching to boost patient adherence.
Collaborative Patient Navigation
- Partner with physicians, care coordinators, social workers, and behavioral health specialists.
- Help navigate community-based resources for nutrition, mental health, or social support services.
- Make timely referrals and track follow-ups to ensure continuity of care.
Reporting & Documentation
- Maintain accurate electronic health records (EHRs) in compliance with policy and standards.
- Analyze patient trends and report on population-level health outcomes.
- Participate in quality improvement initiatives.

Required Qualifications
Education and Licensure
- Current and unrestricted Registered Nurse (RN) license in your state of residence.

Experience

- Prior experience in a remote care coordination or telehealth nursing capacity is advantageous.
