About the Role

Key Responsibilities

Preventive and Proactive Care

• Conduct telephonic or virtual assessments to identify high-risk individuals.
• Educate patients about preventive measures, screenings, and the management of chronic conditions.
• Guide members through lifestyle changes that promote long-term health.

Individualized Health Support

• Create and adjust care plans in response to evolving patient needs.
• Foster understanding by using patient-friendly language and visual aids when appropriate.
• Provide motivational interviewing and coaching to boost patient adherence.

Collaborative Patient Navigation

• Partner with physicians, care coordinators, social workers, and behavioral health specialists.
• Help navigate community-based resources for nutrition, mental health, or social support services.
• Make timely referrals and track follow-ups to ensure continuity of care.

Reporting & Documentation

• Maintain accurate electronic health records (EHRs) in compliance with policy and standards.
• Analyze patient trends and report on population-level health outcomes.
• Participate in quality improvement initiatives.

Required Qualifications

Education and Licensure

• Current and unrestricted Registered Nurse (RN) license in your state of residence.

Experience

• Prior experience in a remote care coordination or telehealth nursing capacity is advantageous.
