About the Role

This position is tailored for experienced nurses who possess a strong foundation in chronic disease management and are comfortable providing support through remote platforms. Key Responsibilities

  • Engage with patients via phone, video, or secure messaging to assess symptoms, understand challenges, and evaluate progress.
  • Develop evidence-based care plans for patients with chronic conditions such as diabetes, hypertension, COPD, and heart failure.
  • Educate patients on lifestyle changes, medication adherence, and health monitoring.
  • Identify risk factors and intervene early to reduce hospitalizations and emergency room (ER) visits.
  • Coordinate care with physicians, specialists, and allied healthcare professionals.
  • Monitor and update patient records using digital health platforms and electronic health records (EHRs)

  • Proficient with digital tools, EHR systems, and virtual communication platforms
  • Bachelor of Science in Nursing (BSN) preferred, but not required
