Key Responsibilities That Center the Patient

Comprehensive Care Planning

  • Develop individualized care plans using clinical judgment, evidence-based guidelines, and patient preferences.
  • Assess health goals, identify risks, and coordinate care for patients with chronic, acute, or post-operative conditions.

Holistic Case Management

  • Conduct outreach calls to assess well-being, medication compliance, and care plan adherence.
  • Facilitate referrals to specialists, behavioral health providers, and community resources.
  • Monitor progress through virtual check-ins and track milestone achievements.

Patient Advocacy and Empowerment

  • Educate patients about conditions, preventive practices, and care options
