Remote Behavioral Health Case Manager

Description

Remote Behavioral Health Case Manager

Imagine the Change You’ll Make

Picture this: a parent logs off a call, finally feeling hopeful, or a young adult finds a path forward after months of overwhelm—all because you showed up, listened, and navigated them through a complex world of care. As a Remote Behavioral Health Case Manager, every day becomes a new opportunity to turn confusion into clarity, connect the dots for people in crisis, and bring relief where it matters most. Here, your mix of empathy, clinical insight, and creative problem-solving transforms daunting journeys into stories of progress and resilience.

Your Impact, Amplified

Your mission? Build more than care plans—build bridges. Every client brings a unique story, and you meet them where they are, both literally and emotionally. You’ll develop strategies that help them not only manage symptoms but truly reclaim their power. Think of yourself as a steady guide and advocate—removing barriers, providing encouragement, and celebrating victories that might otherwise go unnoticed. Whether you’re untangling insurance issues, linking someone to a virtual support group, or de-escalating a moment of distress, you’re the reassuring voice and expert navigator families rely on.

What You’ll Do (and Why It Matters)

  • Craft care strategies tailored to each client’s strengths, not just their diagnoses—because you see people, not paperwork.
  • Conduct behavioral health assessments virtually, using curiosity and active listening to get at what truly matters to your clients.
  • Serve as the connective tissue between therapists, medical teams, social support, and the client, ensuring no one falls through the cracks.
  • Analyze results and trends in real-time, adjusting approaches so clients continue to move forward, even when life throws curveballs.
  • Empower clients to voice their needs, preferences, and hopes, turning every session into a shared journey, not a checklist.
  • Use tech tools and remote platforms seamlessly—so support is just a click away, whether it’s a video call or a secure message.
  • Track milestones in ways that matter to real people, not just numbers—spotting progress, reframing setbacks, and keeping hope alive.
  • Make mental health information accessible, so clients and families understand next steps, not just “medical speak.”

How You’ll Work (and Thrive)

Your digital workspace is built for both flexibility and genuine connection. The tech stack—secure video platforms, cloud-based records, collaborative chat—means you’re always equipped to deliver exceptional care without being bogged down by clunky systems. Daily, you’ll find yourself switching between focused client work, creative brainstorming with your remote team, and personal development sessions designed to keep you energized and inspired. Here, we move fast—but never at the expense of thoughtful, client-centered work.

Who You Are

You’re the kind of person who can make a client feel heard on their hardest day—whether you’re speaking on the phone, over Zoom, or writing a reassuring message late at night. You blend clinical expertise with emotional intelligence and genuine curiosity. Fast-changing circumstances don’t throw you—they energize you. You’re proactive, inventive, and never satisfied with “good enough.” You make collaboration feel effortless and consistently seek ways to grow, both for your clients and yourself.

What You Bring

  • Master’s in Social Work, Counseling, Psychology, or a closely related field.
  • Current clinical license (LCSW, LPC, LMFT, or equivalent) and a strong record in evidence-based behavioral care.
  • At least three years managing behavioral health cases, ideally including virtual care and remote collaboration.
  • Fluency with online documentation, telehealth, and digital communication tools—whether it’s keeping notes organized or leading a virtual family meeting.
  • An instinct for making the complicated simple—breaking down next steps for families, clarifying options, or supporting a team member over chat.
  • Experience collaborating across professional backgrounds to build truly client-focused solutions.
  • Bonus points for bilingual skills, trauma-informed credentials, or demonstrated cultural humility in your practice.
