• Manage a remote caseload of patients with chronic conditions including diabetes, hypertension, asthma, and heart disease.
• Conduct assessments and follow-ups via phone or video to track symptoms, medication adherence, and care plan progress.

Care Coordination

Cross-Functional Collaboration

• Collaborate with primary care providers, specialists, pharmacists, and behavioral health teams to develop and execute personalized disease management plans.
• Support transitional care to reduce hospital readmissions and promote preventative strategies.
• Assess non-clinical factors that influence wellness, such as housing or access to nutritious food, and refer patients to relevant local services for additional support.

• Maintain accurate, timely, and comprehensive clinical documentation within the electronic health record (EHR) system.
• Adhere to HIPAA regulations and telehealth protocols to ensure data integrity and patient privacy.

Required Experience

Licensing and Practice
