🏥 Role Overview

As a Telehealth Diabetes Nurse Educator, you will provide virtual diabetes education, lifestyle coaching, and support to patients managing diabetes. You will work closely with a multidisciplinary healthcare team, utilizing technology-driven solutions to analyze patient data, deliver proactive care, and support patients throughout their self-management journey.

📋 Responsibilities

🔹 Patient Education & Support

• Provide evidence-based diabetes self-management education (DSME) to patients in a virtual environment
• Coach patients on insulin usage, continuous glucose monitoring, carbohydrate counting, and lifestyle modifications
• Educate patients and caregivers on the use of CGMs, insulin pumps, and other digital health applications
• Monitor patient adherence and provide continuous guidance on medication adjustments and lifestyle interventions
• Troubleshoot telehealth-related patient issues to ensure a smooth virtual healthcare experience

🔹 Clinical Collaboration & Data Utilization

• Analyze real-time glucose monitoring data to make informed recommendations on insulin therapy and lifestyle changes
• Collaborate effectively with the clinical team to facilitate smooth coordination of care and timely interventions
• Leverage predictive analytics and remote monitoring tools to identify at-risk patients and intervene early and proactively
• Participate in interdisciplinary virtual meetings to discuss patient progress and refine treatment plans

📌 Qualifications & Experience

✅ Required

• 🏥 Active RN license in the U.S. with multi-state licensure preferred
• 🎓 Certified Diabetes Care & Education Specialist (CDCES) or eligibility for certification
• 💻 Experience using telehealth platforms, remote patient monitoring tools, and digital health applications
• 🏆 Minimum of 3 years of experience in diabetes education, preferably in a virtual healthcare setting
• 📡 Familiarity with continuous glucose monitoring systems, insulin pump therapy, and digital therapeutics
• 📊 Proficiency in interpreting AI-driven patient data analytics and applying it to clinical decision-making
• 🗣 Strong communication and patient engagement skills, with a focus on delivering a seamless virtual care experience

• 🎓 Bachelor’s or Master’s degree in Nursing, Public Health, or a related field
