What You'll Be Doing

Triage and Care Coordination

  • Field incoming calls from patients, families, and caregivers during assigned shifts, delivering calm, clear guidance.
  • Assess symptom severity and make swift, informed triage decisions using hospice care protocols.
  • Recommend appropriate interventions, including medication changes, urgent nursing visits, or caregiver instructions.
  • Escalate high-priority issues to the on-call hospice team as needed.
  • Document all interactions thoroughly and compliantly in the electronic health record (EHR).

Emotional and Educational Support

  • Offer education and reassurance to caregivers navigating difficult hospice decisions.
  • Normalize the dying process with compassion, dignity, and cultural sensitivity.
  • Provide grief support and validate caregiver concerns without clinical detachment.

Collaborative Hospice Environment

  • Engage in virtual huddles with fellow nurses, social workers, and chaplains.
  • Report patterns or concerns to improve care planning and patient outcomes.
  • Actively participate in ongoing education around hospice trends and care standards.
