As a Remote Hospice RN Triage Nurse, you will be the first point of compassionate clinical contact for patients and families navigating end-of-life care. This critical remote nursing role provides timely, empathetic triage services, helping patients manage symptoms, receive appropriate care interventions, and remain supported during times of uncertainty. Key Responsibilities
Patient-Centered Triage Care
- Respond promptly to inbound patient and caregiver calls regarding symptoms, medications, or emergencies.
- Assess and prioritize clinical needs using hospice-specific triage protocols.
- Coordinate interventions, such as nurse visits, medication adjustments, or urgent symptom management, based on the patient's condition.
- Deliver clear instructions to family caregivers on comfort care best practices.
Clinical Documentation & Compliance
- Accurately document all interactions and nursing assessments into EMRs in real-time.
- Ensure triage records comply with hospice regulations and HIPAA standards
- Flag changes in condition for an appropriate interdisciplinary team response
Team-Based Communication
- Collaborate with case managers, physicians, social workers, and spiritual counselors to update care plans.
- Alert field nurses or supervisors when an on-site evaluation is necessary
- Participate in virtual case conferences to remain aligned with patient needs and organizational goals
Hospice Education & Advocacy
- Educate families about the hospice philosophy and symptom management options.
- Provide emotional support to patients and families coping with grief or caregiver stress.
- Promote comfort-first care principles to align with end-of-life goals
Work Environment
This is a fully remote position. You will work from the comfort of your home while providing real-time clinical decision-making. While the work is virtual, the emotional connection and impact you have on families are deeply human.

Required Qualifications
- Registered Nurse (RN) license, active and unencumbered, in your state of residence
- Minimum of 2 years of clinical RN experience, with at least 1 year in hospice, palliative, or critical care
- Proficiency in telephone triage, symptom assessment, and crisis management
- Excellent verbal communication, listening, and clinical judgment skills
- Comfortable using EMRs and telehealth tools in a remote care setting
- Ability to work independently while maintaining close collaboration with an interdisciplinary team
- Strong emotional resilience and empathy when dealing with terminal illness and family stress
Preferred Attributes
- Certified Hospice and Palliative Nurse (CHPN) or willingness to pursue certification
- Experience in remote nursing roles or nurse advice lines
- Prior use of hospice-specific triage systems or symptom management protocols
- Bilingual communication skills are a plus
